What’s Included:
- 2 Comprehensive exams
- 2 Dental cleanings
- 1 set of diagnostic x-rays
- 2 fluoride varnish treatments
- 15% discount on dental procedures
Benefits are effective immediately upon signing up, and for the 12 months thereafter until the last day of the 12th month, or benefits are forfeited. The patient is responsible to schedule and keep both dental cleanings within the first 7 months or the covered benefits are forfeited.
